SERVICES RENDERED
Dr. Tomeski's practice is that branch
of medicine dealing with the diagnosis and nonsurgical treatment of diseases.
This field, known as internal medicine, enables the physician to provide
primary health care, counseling and health maintenance, as well as care for
serious complex health problems of the adult. Dr. Tomeski, in addition,
has subspecialties in hematology (treating blood disorders) and medical
oncology (non-surgical treatment of cancers).
OFFICE HOURS
The office is normally open from 9
a.m. to 5 p.m., Monday through Friday.
APPOINTMENTS
Please phone for an appointment to
assure that the doctor will be available for your visit. When making
an appointment, please give the nature of your visit over the phone so we
can be prepared for your needs and allocate adequate time for you.
CANCELLATIONS
If you cannot make a scheduled appointment,
please telephone and cancel at least 24 hours in advance.
EMERGENCIES
Call the office phone (561-392-6226)
for emergencies at anytime. The medical assistant or answering service
will contact the doctor, usually within a few minutes. If the emergency
is extreme, the use of the "911" emergency number may be appropriate,
or you might go directly to the Hospital Emergency Room and have the doctor
contacted. If the doctor is away, she will have made arrangements for
a qualified physician to substitute for her.
HOSPITALS
Dr. Tomeski is on the medical staffs
of Boca Raton Community Hospital (561-395-7100), Delray Community Hospital
(305-498-4440), and West Boca Medical Center (561-488-8000). In the
event that you need to be hospitalized, this office will make appropriate
arrangements for you.
FEES
All fees are carefully established
and meet the norms of Medicare and other medical offices in the area.
Sound business and collection procedures are essential to continue operation
and maintain high quality services for you.
Office Visits: Payment is due at time services are rendered. In-Hospital
Doctor Services: Payment is due when billed,
and charges should be paid in full within 60 days of discharge.
Patients with problems in paying doctor's fees should discuss them as early
as possible, and arrange an acceptable payment schedule.
Accounts which are more than 60 days past due are considered to be delinquent
unless other arrangements are made with this office.
INSURANCE
Unless otherwise arranged with this
office, we must expect payment directly from the patient. Your
insurance contract is between you and the insurance company and/or government
agency. Only you and your insurer can resolve differences arising over a claim.
This office will provide you with the necessary data (super bill) to supply
to your insurance company or government agency. It is the patient's
responsibility to file claims and to seek reimbursement from the insurer.

PATIENT'S RESPONSIBILITY
Health care is a reciprocal relationship
between patient and physician. The patient is expected to follow the
advice given by the physician, and to provide the physician with accurate
information about medical history, etc. Also, the patient is expected
to keep his/her account current to enable the office to maintain a quality
service. The patient should communicate with the office about relevant
problems, changes of address, etc. The physician cannot assume medical
responsibility for patients who do not carry out their obligations
