23K PROFESSIONAL SERVICE
Be it known that the funeral home named above hereby guarantees their professional services, that part shown on the purchase agreement on service date and contracted for by the Informant.
If the above individual is not satisfied with some aspect of our professional services, we will return that portion of the professional services fee as indicated on the General Price List, if said individual gives written notice of dissatisfaction within seven days of the provided funeral service.
