Do not eat or drink anything after midnight of the night before
surgery.
2.
If you are having a breast procedure and are 35 years of age
or older, a mammogram is required. If you have had a mammogram
done within 12 months from time of scheduled surgery, our
office must receive a copy of the result no later than 10
days prior to the surgery date. Breast cancer patients are
required to bring a copy of their mammogram report, ultrasound,
and/or pathology report at time of consultation. This information
will be made a part of your chart.

If you are 55 years of age or older, medical clearance must
be obtained from your primary care physician. Our office must
receive a copy of your medical clearance results 10 days prior
to the scheduled surgery date. Patients under 55 years of
age must undergo required laboratory tests at an appropriate
hospital facility.

Appropriate photographs must be taken by one of our physicians
or staff photographer prior to surgery date. Consent forms
must be signed prior to surgery date. Payment is due 10 days
prior to surgery. Cancellation is subject to a $300 processing
fee. You will need to arrive at the hospital approximately
2 hours prior to scheduled surgery time.
5.
You must call your surgical facility 10 to 12 days prior to
surgery for preoperative instructions regarding medical clearance,
labs, and payment. Please contact your respective facility
