A limited number of work-ins are seen each day, especially when there
are cancellations. Not everyone requesting a work-in appointment can
be accommodated on the same day.

If you have to cancel, please do so as soon as possible, at least
24-hours prior to the scheduled appointment.
After the working hours, for allergy and asthma emergencies, the doctors
can be reached through the 24-hour answering service by calling 225-379-6810.
They will contact the doctor through a pager system. Non-emergency
calls and routine prescription refills may be done during regular
business hours.
We participate with most of the insurance carriers including Blue Cross Blue Shield of LA (All plans), United Healthcare, Aetna, Office of Group Benefits, and all private insurances. We also see patients on Medicaid and Medicare
with referrals from the primary care physicians.
Please bring your insurance information, including the ID cards and
written referral from your HMO or PPO on the day of the appointment.
During your follow-up visit, provide information about any insurance
changes. If you have not updated your demographics and insurance information
within a year, you will be requested to do so.
Wear casual clothing for your appointment, especially if you are undergoing
allergy skin testing. Skin tests are done on the forearms, upper
arms, and at times on the upper back.
Please arrive 15 minutes prior to your scheduled appointment. No two
patient's visits are exactly the same. Depending on the nature of
the problem, a new patient visit could last 1 to 3 hours and follow-up
visits could last from 15 to 45 minutes.
If you are scheduled for allergy skin testing, do not take any antihistamines,
including the over the counter ones, for at least five days prior
to the appointment. Tricyclic and Tetracyclic antidepressants should
be stopped for at least 2 weeks prior to the appointment. If you suspect
that you may be on an antihistamine, check with your pharmacist. Do
Not Stop Your Heart, Blood Pressure or Asthma Medications.
| Payment
and Collection Policy |
Payment is due when the service is rendered. The patients are expected
to pay deductibles and co-pays and fees for any non-covered benefits
on the day of the visit. AAIC will file the insurance for all carriers
that we are participating providers of. Payment for allergy injections
(unless covered by your insurance company) also should be paid on
the date of service or prior to the service. Bills should be paid
upon receipt. For repeated billing, re-billing charges will apply.
Please feel free to ask any questions about your diagnosis, procedures,
laboratory tests or the treatment plan, including immunizations and
allergy injections and their schedule. If, at any time after your
visit, you have questions or concerns, please do not hesitate to call
us back. We welcome the opportunity to serve you.
Medical records are strictly confidential. Be aware that these are
subject to periodic quality control review by your insurance. Without
your authorization, we can not and will not release them to anyone.
With your consent, we will be happy to share the records with other
physicians. All managed care insurance carriers require that we communicate
routinely with your PCP's. Record requests should be made as far in
advance as possible.

| Allergy injections are given from 8:00 am until 6:30 P.M. You are required to wait 30 minutes after receiving allergy injections. Allergy injections are not given on the days of follow-up visits with the doctor. |
